- Entrants who make a purchase at Walmart.com, Walmart.com/grocery, a participating Walmart retail location, or any Walmart mobile apps or websites during the Sweepstakes Period and receive a survey invitation via e-mail (the “Invitation”) must visit the unique survey URL (the “Survey Website”) found in the Invitation to complete the survey.
- The online survey and/or entry form may be filled out by participants who get a survey invitation on their cash register receipt after making a purchase (of any amount) at a participating Walmart retail location during the sweepstakes period.
- Within one (1) week of the cash register receipt date, the entrant must log on to www.survey.walmart.com (the “Purchase Website”) and submit the unique code.

- Don’t have your receipt? No problem, click here to share your feedback.
- Answer one by one, all the questions on the survey.
- Complete the survey in its entirety and click on the “Submit” button.
- Entrants may be prompted to complete the entry form in its entirety to include first name, last name, street address, city, state, zip, phone number, and email address (if available) and click “Continue”.
- If prompted, the entry form must be completed in order to be eligible to win. Then, complete the checkboxes and click “Submit” to receive one (1) entry into the Sweepstakes.
Surveys must be completed within one (1) week from the cash register receipt date and by 11:59:59pm CT on July 29, 2022 if the cash register receipt date is July 22, 2022 or later.
