• Only legal residents of the United States, Puerto Rico, or the District of Columbia are eligible to participate.

• The survey code is valid for 7 days after you receive your receipt. Maximum of one survey per household per month.
• The survey code on your receipt can only be used once. To receive your discount, you must redeem your coupon within 90 days of completing the survey. There is no cash alternative.
